Name, Phone, Class Day,  Date ,Time and Location of Makeup

Please check your spam and junk mail in case you don't receive your email confirmation.

Please note:You are allowed 2 makeups per semester on a First Come, First Serve Basis if you miss your makeup, then your makeup will be forfeited.
